K-12 OPEN ENROLLMENT APPLICATION, 2012-2013 SCHOOL YEAR

You are submitting an application after the Open Enrollment deadline has passed!

This application will be marked “Late Applicant” and will not be included in the lottery process.

Late Applications are always sorted by date/time based on the order they are received

and will fall to the bottom of any existing wait list as they are received.

Please submit one application per student, not one per family. Multiple applications for a

single student will be deleted.

Kindergarteners must be five years old, and first graders must be six years old by September

30, 2012
